Appointments

Please arrive 15 minutes prior to your appointment time, as this will allow staff to check your current information, copy your insurance card and obtain photo identification. If for any reason you are unable to keep your appointment, please give us 24 hours notice, so we can provide that time for someone with urgent care needs.

Late Arrival

If you arrive more than 5 minutes past your scheduled appointment time it may affect your ability to be seen by a provider. Late patients may be given the option to wait for another appointment time on the same day if one is available or you may be asked to reschedule. If you are repeatedly late or fail to show up, without calling to notify us, we may not be able to continue to provide care to your children.

No Show

When a family fails to show for 2 appointments, the family will be notified by mail. Repeated no show appointments could result in termination from the clinic.

Telephone Calls

Our staff makes every effort to provide excellent telephone service to our patients. We take routine calls from 8:00 am to 5:00 pm and urgent calls from 5:00 pm to 9:00 pm. Legacy Emmanuel triage nurses are available for emergency calls after 9:00 pm. Families can call the main SPC number at 503-362-2481 and the answering service will direct your call appropriately. There are times when the phone call volume is exceedingly high, so please be patient with our staff. We will call you as quickly as possible.

Health Insurance

All families with insurance will be required to bring their current insurance card with them to the visit. This includes commercial and Medicaid cards.

Leaving the Practice

It is our policy to provide care up to the age of 18 years or until the patient graduates from college. Exceptions to this policy need physician approval. We recommend patients leaving our practice sign a medical record release to the new physician as soon as possible.
